FQHC sliding fee

Mountain Park is a Federally Qualified Health Center that provides primary care, OB/GYN, pediatrics, dental, behavioral health, pharmacy, WIC and wellness programs. They accept individuals on Medicare/Medicaid  (AHCCCS), have a sliding fee scale for uninsured patients and accept private insurance. They are a Well Woman HealthCheck provider.
